Ask your employer for a confirmation of tax payment. Entrepreneurs and persons filing a tax return of type A or B fill in the recipient of the 2% tax directly in the tax return in the Declaration on remittance of the share of income tax paid.

Note, that you need to be employed in Slovakia.
